Health & Wellness Director (RN or LPN)

We are currently seeking a Health & Wellness Director (HWD) to join our dynamic and mission-driven leadership team. This is a full-time, salary-exempt position overseeing all aspects of the Health & Wellness Department in both our Assisted Living and Memory Care neighborhoods. What You'll Do:

• Lead and support a team that includes a Resident Care Coordinator, Memory Care Coordinator, Medication Assistants, and Caregivers.
• Ensure exceptional care and services are provided in alignment with individualized service plans and applicable state regulations.
• Oversee documentation, assessments, and charting practices for accuracy and compliance.
• Provide oversight and support to the Resident Care Coordinator and Memory Care Coordinator in managing caregiver and med tech staffing, ensuring appropriate coverage is maintained 24/7. While scheduling duties are delegated, the Health & Wellness Director holds overall responsibility for ensuring staffing levels meet the needs of the community at all times.
• Collaborate with residents, families, and care providers to support overall wellness.
• While this role does not provide direct nursing services, you will lead your non-licensed team to observe, document, and report changes in resident condition, facilitating prompt attention from residents' healthcare providers.
• Partner with the Executive Director and other department heads to ensure strong communication and a resident-centered approach to care.
• Participate in the Manager-on-Duty (MOD) weekend rotation and maintain a flexible schedule based on the needs of the community.
• Maintain 24/7 on-call responsibility for the Health & Wellness Department to ensure urgent care, staffing, or resident safety concerns are addressed promptly. What We're Looking For:
  • An active RN or LPN license in good standing with the State of Arizona.
  • An active Arizona Fingerprint Clearance Card, TB clearance, and current CPR & First Aid certification.
  • A valid Food Handler Permit (or willingness to obtain upon hire).
  • At least 2 years of experience in a healthcare leadership role is preferred, though strong candidates with transferable experience will be considered.
  • Strong interpersonal, organizational, and problem-solving skills.
  • A calm, professional presence-especially in high-stress or urgent situations.
  • A passion for working with seniors and a desire to lead with purpose and heart.
