  • Candidates with an AZ home address on their drivers license will not be considered. Please include a copy in submission.
  • Banner has a specific cover page! PLEASE vet all locations to the travelers and add all experienced departments candidates are willing to ACCEPT/work in.
  • Start Date should be on or very quickly after the Order Start Date listed.
  • All RTO should be listed on the cover page, must be less than 7 days.
  • RTO will not be approved in the first 2 weeks of assignment.
  • All facilities the traveler is open to working in should be listed on the submission.
  • If interested in 'any' location, please include what states (I.E. Any AZ, Any CO, Any state, etc.)
  • All shifts the traveler is open to working on should be listed on the submission.
  • All departments the traveler is open to working must be checked on the cover page.
  • All candidates submitted should be preclosed and ready to accept.
    • Job Requirements
    • 24 months minimum of current ICU experience - REQUIRED
    • Certifications must be American Heart Association OR American Red Cross, active upon submission and INCLUDED IN SUBMISSION - REQUIRED
    • No Nursys Hits - REQUIRED
      • Additional Details
      • Flu vaccination - Banner declinations can be applied for (reach out for forms). Please add vaccination status in the submission notes.
      • Banner has very specific background requirements that must be adhered and attested to. Please look at the compliance requirements immediately.
      • Travel experience - Preferred
      • Profile requirements include: Banner Cover Page completely filled out, Resume with all facilities, facility size, beds, trauma level, and units listed, Skills Checklist specific to the discipline (must be less than a year old), Active License (verification must be less than 30 days old), Driver's License.
      • Banner assesses a 2% payment discount fee and a 2% technology fee on all transactions.
      • Learning modules must be completed prior to start of assignment.
      • Client or Vendor may cancel an Assignment in progress with 7 days written notice without penalty.
