Registered Nurse (RN) - Case Management (Utilization Review) Job
Specialty: Care Coordinator - Discharge Planning Job Duration: 13
weeks Shift: 10-hour shifts: Friday-Saturday-Sunday-Monday OR
8-hour shifts: variable days rotating weekends Guaranteed Hours: 40
hours per week Experience: Minimum 2 years License: Yes, willing to
wait Certifications: Basic Life Support (BLS) required ACM
(Accredited Case Manager) or CCM (Certified Case Manager) preferred
Must-Have: Strong complex discharge planning experience in inner
city teaching hospital settings Previous travel experience
preferred Job Description: - Responsible for case management and
discharge planning for inpatient and emergency department settings.
- Handle a caseload of 32 patients per day for MedSurge and 48 for
ICU. - Collaborate with RT and pharmacy departments for discharge
planning. - Work predominantly with MedSurge, transplant, and neuro
patients. - Utilize the Cerner EMR system for documentation. -
Participate in department-specific orientation of 1 to 2 weeks. -
Adhere to business casual dress code or wear pewter/dark grey
scrubs. - Ability to self-schedule and accommodate block
